Default advice on very silent barebone

Anybody have experience with very silent barebones (available in belgium), needed for office use, integrated graphics are ok, no need for hi-power use, no overclocking.
just needs to be SILENT

Buy a shuttle with icy cooling.
Put a very low voltage CPU on it. THe mainboard controlls the fan itself, and when not running hot, it's silent as hell

Will this be silent?

(if it folds, will it be loud as hell? )

If I make a pc with a centurion coolermaster, an amd 64 3000+winny, onboard gpu and one zalman 80mm, ocz psu 420w, it's only 50 euros more, won't that one be just as silent (but bigger)? (even if it folds...), it just has the ocz psu (silent!), one zalman slow speed fan, and the standard cpu cooler +cool'n'quiet...

It's not for me, for my mother-in-law her medical office.
